A Case of Headache Attributed to Spontaneous Intracranial Hypotension Misdiagnosed as Aseptic Meningitis

Abstract

|
Spontaneous intracranial hypotension (SIH) is characterized by throbbing headache occurring or worsening in the orthostatic position and relieved by lying down. Low cerebrospinal fluid pressure and diffuse pachymeningeal enhancement on brain magnetic resonance image (MRI) are the major features of the SIH. When CSF findings are not characteristically compatible with meningitis and when the headache is particularly orthostatic in nature, SIH should be included in differential diagnosis. But previous reports suggest that SIH can present with variable clinical features other than orthostatic headache. MRI and radioisotope cisternography findings are clearly contribute to the differential diagnosis between aseptic meningitis and SIH. We report a patient diagnosed with SIH which initially misdiagnosed as aseptic meningitis.
